Is New Beginnings Behavioral Treatment Agency the right fit?
Before you begin the application process there are several factors that will help you determine if New Beginnings has the right treatment program to help your child achieve long-term success.

What level of care does your child need? Residential Treatment, Partial Hospitalization Program (PHP)/Day Treatment, or Intensive Out-patient Treatment

Please go to the New Beginnings Parent Portal to access and complete the admissions application.  Please fax clinical  and supporting documents (assessments, clinical notes, psychiatric evaluations, etc.) to 801-855-7215 or email to admissions@newbeginningslv.org.

Referring Providers:  If you are a provider of an Optum Salt Lake County consumer all higher level of care services (RTC and Day Treatment) require a referral and prior authorization.  Please download and complete the below referral from using Microsoft Word and submit the completed form directly to femia@newbeginningslv.org for program admission review.  In addition, please send a copy of the completed referral form to utclin@optum.com for service authorization.  OptumSLCo will notify the referring provider directly of a decision within 1-3 business days.
